Medical Programs (1 page)

The form is mandated by Wilderness Medical Associates and is required for participants attending the Wilderness First Responder (WFR) AND/OR Medical Service programs — all parents must sign this and the participant must bring this with them to the program. Dominican Republic: Diver’s of the Caribbean PADI Scuba Dive Forms (5 pages)

Required for participants intending to dive on the aforementioned program. Some pages do require a medical exam from a doctor as scuba diving has special eligibility (health) requirements. Additionally, if you answer “yes” to any of the medical requirements on page 4 you must have a physicians sign off on the last page. You must bring this form on the program with you.

New Orleans: St. Bernard Project Forms

Required for participants attending the New Orleans Rebuilding program — the Country Director will send a link to all of the parent emails listed to the online waiver form a week before the departure date.

Directions

  • Copy and paste the link into your web browser. Leave the Volunteer Information (top section) of the form AS IS. This is pre-populated with information specific to our group!
  • The rest of the form can be filled out with the information about your student. Once the form is complete and signed, the form is sent to St. Bernard Project and will be on file in their system. The Country Director will receive a list of students from St. Bernard Project prior to the trip starting to confirm forms are on file.
  • Before the participants arrive in New Orleans, the volunteer coordinator at St. Bernard Project will notify the Rustic Pathways program managers of any participants who have yet to complete their online form.

Cuba: Required Consent to Travel Statement

Students traveling to Cuba are required to complete two copies of a Consent to Travel Statement, this document must be notarized and a hard copy must be returned to us by mail. The student should travel with the other copy on their person when departing for and returning from their program.

All Other Countries: Optional but Highly Recommended Consent to Travel Statement

The U.S. Customs and Border Patrol as well as other international organizations highly recommend that children under the age of 18 traveling without a legal parent / guardian travel with a Parental Consent / Permission Letter. This is an optional letter, that while not required is also highly recommended by Rustic Pathways for participants under the age of 18. Should you wish to create a Consent to Travel Statement, the participant should keep a copy with them while traveling. This document does not need to be returned to Rustic Pathways.
